As part of your enrollment, Massachusetts State Law requires that incoming students provide proof of immunizations and health insurance. Also, Regis College requires completion of additional health forms prior to arriving on campus. These documents must be uploaded through our secure portal as outlined below.

Who needs to submit health forms:

  • all full-time undergraduate students under 30 years of age;
  • all full-time graduate students under 30 years of age;
  • all full- and part-time nursing and health science students regardless of age.
  • The above categories include individuals from another country attending or visiting classes or educational programs as part of an academic visitation or exchange program.

Health Forms

Health forms must be electronically uploaded to the Regis Center for Health and Wellness via a secure site no later than:

  • Tuesday, July 15, 2025, for fall enrollment
  • Monday, January 5, 2026, for spring enrollment

Instructions for Downloading, Completing, and Uploading Required Health Forms

Step 1: Download the Health Forms

  • Click on the appropriate box provided below, to download the PDF file, save it to your device or print out the forms to be completed.

Step 2: Complete the Health Forms

  • Save your completed forms to your device that you are going to use for the uploading process. This can be done by either scanning each page into one PDF document or by taking a clear, portrait-oriented picture of each page. Files must be in either PDF or JPEG format types.

Step 3: Upload the Health Forms

  • Click on the appropriate box provided below.
  • Click on the “select files” box to select your files for upload. Once you attach your files, you will be required to enter your first name and last name. You will then click on the “upload” box.
  • When 100% of your forms have been uploaded, you will see a green circle with a check mark and your screen will show, “finished uploading”.

Step 4: Confirm Submission

By following these steps, you’ll be able to efficiently download, complete, and upload your health forms.

Note: Incoming undergraduate and graduate students for the fall 2025/spring 2026 school year will be notified by email when the website becomes available for uploading of Health Forms.

Medical and Religious Exemptions

Students, faculty, and staff who have a qualifying medical condition, disability, or who object based on a sincere religious belief, practice, or observance may be exempted from vaccination requirements pending completion and review of exemption form submission.

Required Immunizations

Massachusetts State Law requires that incoming students provide proof of immunizations. Requirements apply to all full-time undergraduate and graduate students under 30 years of age and all full and part-time nursing and health science students regardless of age. This includes individuals from another country attending or visiting classes or educational programs as part of an academic visitation or exchange program. Health information must be submitted no later than Tuesday, July 15, 2025, for fall enrollment and Monday, January 5, 2026, for spring enrollment.

VaccineRequirements
Tdap (Tetanus-Diphtheria-Pertussis)One dose; and history of DTaP primary series or age-appropriate catch-up vaccination. Tdap given at ≥ seven years may be counted, but a dose at age 11 - 12 is recommended if Tdap was given earlier as part of a catch-up schedule; Td or Tdap should be given if it has been ≥10 years since Tdap.
Hepatitis BThree doses; Laboratory evidence of immunity acceptable; two doses of Heplisav-B given on or after 18 years of age are acceptable.
MMR (Measles, Mumps and Rubella)Two doses; first dose must be given on or after the 1st birthday, and the second dose must be given ≥ 28 days after the first dose; laboratory evidence of immunity is acceptable; birth in the U.S. before 1957 is acceptable only for non-health science students.
Varicella (chickenpox)Two doses; the first dose must be given on or after the first birthday and second dose must be given ≥ 28 days after the first; a reliable history of chickenpox 1 or laboratory evidence of immunity are acceptable; birth in the U.S. before 1980 is acceptable only for non-health science students.
Meningococcal vaccineOne dose; One dose MenACWY (formerly MCV4) required for all full-time students 21 years of age or younger; the dose of MenACWY vaccine must have been received on or after the student’s 16th birthday; doses received at younger ages do not count towards this requirement. Students may decline MenACWY vaccine after they have read and signed the MDPH Meningococcal Information and Waiver form. Meningococcal B vaccine is not required and does not meet this requirement.
  1. A reliable history of chickenpox includes a diagnosis of chickenpox, or interpretation of parent/guardian description of chickenpox, by a physician, nurse practitioner, physician assistant or designee.

Tuberculosis Screening

In addition to complying with the above immunization requirements, students must complete the Tuberculosis Questionnaire and Testing Forms (found in the Health Forms). Based on the answers of the questionnaire, some students may be required to have a tuberculin skin test (Mantoux/PPD) or blood test and have a health care provider complete the Tuberculosis Questionnaire and Testing Forms.

Our Center for Health and Wellness hours during the academic year are:

  • Monday from 9 a.m. to 7 p.m.
  • Tuesday through Thursday from 9 a.m. to 4 p.m.
  • Friday from 9 a.m. to 3 p.m.
    • Closed on Friday for summer sessions

The Center for Health and Wellness is closed on weekends and Regis observed holidays.
